New Patient Portal User

Note: If you provided an email address while registering at Mercy Iowa City, you should receive an email invitation that will provide access to the patient portal. This New Patient Portal User request process is only required for those patients who have not provided Mercy with an email address.

There are three steps required to request access to your Mercy Patient Portal (if you did not give your email address during registration). You must complete all three steps before you will be given access to the Mercy Patient Portal.
  1. Print this form (Mercy Patient Portal Access Application), complete it, and sign it. Please be sure to make email address legible.
  2. Mail or fax the completed form to:

  3. Go to the Mercy Portal Login screen. Click on the "Register" tab (top left under Mercy logo). Click on the patient option and complete the "Register Your Account" fields. Click on "Register" when done.

Your request will be processed within 5 business days. You will receive an invitation email giving you instructions on how to login.

Please note: Information prior to April 2014 is not available in the Mercy Patient Portal.
