First-time users - Set up your personal account:

Received email invitation:

• Click the registration link in the email you receive from noreply@followmyhealth.com
• Create your portal login using a unique username and password
• Enter the invitation code provided by us. Then, follow the on-screen prompts to complete your account

Did not receive email invitation:

• Once your account is created in FollowMyHealth, go to My Account  > Connection
• On the page that opens, choose Add Organization
• Enter the following in the fields listed: Zipcode: 52245, Distance: 10 miles, Organization type: Hospital 
• Once submitted and your information is verified, you will receive a confirmation email
• Link to FollowMyHealth Patient Portal
  

Already have a FollowMyHealth account?

• Click the link in your invitation email and select “Sign in and add this connection.” This will keep all of your health information in one place.

I am a representative (proxy) for my child or dependent adult. To get started: 

• Parents and guardians can request to be set up as an authorized individual for their children.  Please complete the FMH Authorized Individual Request Form and email it to patientportaladmin@mercyic.org, or mail it to:

• Once the request has been confirmed and approved, you will receive an email from FollowMyHealth to allow this connection.
• Click the link at the bottom of the email you receive from noreply@followmyhealth.com.
• Once FollowMyHealth opens, choose Sign Up if you do not have a current FMH profile, or choose Sign In if your profile already exists.
• Enter the invitation code provided by your medical organization and click "Agree" to the release of information. The invitation code will be the 4-digit year of your birth.
  
  *Please note: Parent/Guardian access will be limited once the patient reaches the age of 14 and revoked when the patient turns 18. Patients aged 14 or older are encouraged to sign up for FollowMyHealth to begin to manage their own health records.

I am a representative (proxy) for an independent adult. To get started: 

• Please complete the FMH Authorized Individual Request Form. This form must have a written signature from the patient. In lieu of the patient's signature, legal documentation (Such as a Power of Attorney) will be required.
• Once the form is complete, email the signed form to patientportaladmin@mercyic.org, or mail to:
